A nurse from the University of Chicago will be here at De La Salle on January 26th from 12-3pm.
Attached is the flyer from University of Chicago with instructions and the phone number to call to schedule a vaccination appointment.
In addition, parents will need to sign a consent form that needs to be brought with the day of vaccine.
