Transcript Requests

Protecting your privacy and the integrity of your academic records is important to us. Because there is no way to verify that an email is actually coming from you, we do not accept requests for transcripts by email.

There are two ways to obtain a copy of your De La Salle Institute or Lourdes High School transcripts:

You can make the request in person by coming to the
Registrar’s Office at 3434 S. Michigan
Send a request with your signature by US mail.

When sending a written request, please include the following:

date of birth
name at time of attendance
year of graduation
social security number
whether the transcript needs to be official or unofficial
where to send the transcript
your contact phone number in case of questions

There is a $5.00 charge for each copy of your transcripts. Be sure to include a check or money order if you send a written request.
